How to get started with services

If you are a new client

The first step to start services with us for any program is to do an initial intake over the phone. The purpose is to understand your child’s needs and discuss service options. To start the intake process, please call Shannon Haggerty at 303-759-1192 ext 10 or email at info@altavista-autism.org.

Following the intake, we will assign you to a member of our clinical team, who will conduct an initial informal assessment with you and your child.  The assessment period is an opportunity for us to get to know your child’s strengths, needs and potential areas of focus, as well as your family’s goals, for your child.   We will ask you to complete an intake packet  prior to the assessment.

During or after the assessment, we will recommend services for your child.  Clients interested in pursuing services will complete the admissions process.

If you are interested in outreach services

If you have completed the initial consultation and are interested in outreach services, please complete the Request for Outreach Services Form and mail or fax back to 303-759-1194 attn Shannon.

If you are interested in therapy services

If interested in individual speech, OT, behavior, or psychology services, please download and complete a Therapy Request Form.
