For Evaluations or Consultation Services:

If you are interested in an evaluation or a consultation regarding your child, please contact us at info@altavista-autism.org or at 303-759-1192 ext 10 to complete the intake process. You may also download the Outreach Request form, complete it, and mail or fax back to us at 303-759-1194.

For School or Professional Consultation or Training:

If you want to learn more about or utilizing our consultation, program development, or training services, please contact us at info@altavista-autism.org or at 303-759-1192 ext 10.   A member of our outreach staff will follow up if services are requested.

For Therapy Services:
